Copy of order of appointment to be served upon protected person

  • notice of entry of order to be filed with court.

1.

A copy of the order appointing the guardian must be served personally or by mail upon the protected person not later than 5 days after the date of the appointment of the guardian.

2.

The order must contain the names, addresses and telephone numbers of the guardian, the attorney of the protected person, if any, and the investigator.

3.

A notice of entry of the order must be filed with the court.
