Application for license

Any person wishing to operate a cosmetological establishment in which any one or a combination of the occupations of cosmetology are practiced must apply to the Board for a license, through the owner, manager or person in charge, upon forms prepared and furnished by the Board. Each application must contain a detailed floor plan of the proposed cosmetological establishment and proof of the particular requisites for a license provided for in this chapter. The applicant must certify that all the information contained in the application is truthful and accurate.


The applicant must submit the application accompanied by the applicable required fees for inspection and licensing. Upon receipt of the application, the Board shall contact the applicant to arrange a date and time to conduct the on-site inspection and to issue and activate the license. A license issued pursuant to this subsection is not valid until it is activated.


The fee for issuance of a license for a cosmetological establishment is:


For 2 years, $200.


For 4 years, $400.


The fee for the initial inspection is $15. If an additional inspection is necessary, the fee is $25.
