Application for business license or license as applicator.

1.

An application for a business license or license as an applicator must be submitted to the Director and must set forth such information regarding the applicant’s qualifications and proposed operations and other relevant matters as required by regulation of the Director. If the applicant is a natural person, the application must include the social security number of the applicant.

The Director may require an applicant for a business license to establish to the satisfaction of the Director that a primary principal who is licensed as an applicator is associated with the business of the applicant.

3.

If an applicant fails to complete the licensing requirements within 30 days after the date on which the applicant submits his or her application, the applicant forfeits all fees he or she has tendered. Thereafter the applicant may reinitiate the application process upon payment of the appropriate fees.
