“Written documentation” defined.

“Written documentation” means:

1.

A statement signed by the attending provider of health care of a person diagnosed with a chronic or debilitating medical condition; or

2.

Copies of the relevant medical records of a person diagnosed with a chronic or debilitating medical condition.
