Form and contents of license.

Each license issued by the Division shall be in the form prescribed by the Division and shall contain:

1.

The name of the person or persons authorized to operate such licensed facility;

2.

The location of such licensed facility; and

3.

The number of beds authorized in such licensed facility, the nature of services offered and the service delivery capacity.
