Preparation and distribution of forms and blanks

1.

The Board shall prepare, print and supply to all local health officers all blanks and forms used in registering, recording and preserving the returns, or in otherwise carrying out the purposes of this chapter.

2.

The State Registrar shall charge for each blank certificate of birth, death or stillbirth a fee of $1.
