Forms for admission.

All applications and certificates for the admission of any person in the State of Nevada to a mental health facility or to a program of community-based or outpatient services under the provisions of this chapter shall be made on forms approved by the Division and the Office of the Attorney General and furnished by the clerks of the district courts in each county.
